One decoding pass

Decodes HTML named references, decimal references and hexadecimal references using HTML text-context rules. Browser-style handling applies to invalid numeric references; unrecognized names remain unchanged. Decoding happens once, so & becomes &. Output stays plain text. Review it before placing it into an HTML page because decoding can restore markup characters.
